"is November a good month to visit Japan? "

Yes. Except possibly Hokkaido. Most of Japan is dry and not to cold at that time and as JCT said, the autumn colours are spectacular. I found Shikoku particularly wonderful in November.

"If yes, then what all places are worth visiting in November?"

All places, except possibly Hokkaido. It depends on why you want to go to Japan in the first place.

Start your research on Japan guide, which has a page on koyo (fall foliage) if interested.
