My personal advice:

1) I don't think you need to book for a guide for Hakone at all.

2) Kyoto two nights seem a bit too short (and you might not have time for Nara); you might cut one night for Tokyo and add to Kyoto.

I would stay in Tokyo only 3 nights initially and stay in Kyoto for 5 nights. See Nara, which means no need to see Kamakura (the real deal is better than the copy). Kyoto is the top tourist destination in Japan for many reasons. Tokyo is not.

I also favor more nights in Kanazawa over Takayama, but your trip.

Skip Kamakura day trip. Add that day to Kyoto so you can do a day trip to Nara. Include Kamakura only if you weren't leaving Tokyo area but since you're going to Kyoto, skip it.

If you are going to Hakone with hopes to see Mt Fuji be prepared that you may not see it due to weather.

I did 1 night Takayama and 2 nights Kanazawa with a stopover for 3 hrs at Shirakawago on my way to Kanazawa.

Kyoto is such an interesting place with lots to see. We stayed there 4 days last year and could easily have spent two more. Try to spend 3 full days in Kyoto if possible.
