You should change USD to Dong to pay for meals or shopping. It will be easier than paying by $. ATM is quite popular in big city..

Many things are advertised in US$, but you have to pay in VNDong. It's the law.

Change in Dong is easier. Vietnamese people prefer to pay in cash than by card. Instead of fast food, you can have many other things to eat in Vietnam, of course, it is cheap.

When you get here change your dollars into Dong. Easy to do, current rate is approx $1 = 23,000 Vietnam Dong. Strange that you have to pay for lunch as that is normally included in the tour. Even stranger that you want to have Mac Donalds :-) If you go out of Saigon / HCMC or Hanoi there will be no Mac Donalds but plenty of other nice Vietnamese or western type foods to enjoy. Cost of a nice simple meal or pepsi will be the same as you pay at home. Enjoy.
