Hotel Porta San Mamolo is a very nice small older hotel. Service was good and food as well. We were part of a group tour and this was our first hotel in Italy. At this time I do not remember details at all.

to reach our Hotel from the Train Station (there is only one main Station Bologna Centrale) it is possible to take a bus (33, 32 or 30 but probably 33 is the easiest one), you can get tickets at the bus tickets office TPER (you find it at the exit on your right side), it will take around 15 minutes to reach Piazza di Porta San Mamolo stop, 2 minutes walking distance from us.
If you don't have heavy luggage it is also possible to walk, crossing the center it takes 35 minutes. By taxi it takes around 10 minutes for 10/12,00 euro.

No, the Porta San Mamolo is not just in the center; it is just at the edge of where the old city wall passed (that's why it is called "Porta" (Gate)). To get to the Piazza Maggiore, you have to walk four long blocks. The area is very nice, however, and there is a good restaurant at the corner called Sangiovese.

We had a hire car to get to the hotel and then walked everywhere. There is a bus map at people.reed.edu/~reyn/BolognaMap.pdf which is a bit complicated but the A bus looks like it goes within a few 100 yards of the hotel.
