I am traveling thru LHR i have a 12 hour layover do i need a visa to get a Yotel or enter one of the lounges where i can relax with out leaving the airport

The Yotel is on the "landside" of the airport. You do have to pass through customs. If you need a VISA to enter the U.K. (Yotel is on the U.K. side of border control, past the arrival gates), you will need a VISA to get past immigrations to get "out" to the Yotel.
You have to fill out the form and pass through customs. They will stamp your Passport. Let them know you are going to Terminal 4.
You do need a visa to get to the YOTEL at LHR
We have US passports and did not need visa, but you'll have to clear immigration. Yotel was perfect for a rest, shower, refresh on our layover at LHR.
It's based land side so you will have to clear immigration first.
I'm a us citizen and I did not. If you are not, check further.

You have to pass border control to get to the Yotel so if you want to spend time at the Yotel visa rules would apply depending on your situation.
