I was on a prepaid golf tour of Vietnam so I cannot answer the first question. Caddie Tip is usually USD15 as caddie fee paid by Club.

it was almost 300 us dollars for 18 holes , clubs, and two sleeves of balls. that included cart and fore caddy, but i did tip her at the end.

Caddies and buggies are mandatory. I took my own clubs so not sure about hire club quality. If choosing between Montgomerie or Ba Na Hills personally I would pick Ba Na Hills every time.
