I don't recommend staying here unless you HAVE to be in Karlovy Vary and there are absolutely no other options. Then, it should be fine.
The Good
The location is quite central, very close to the Grand Hotel Pupp, and the main thermal springs. That said, it's a small town, so most things are that close...
The Bad
The rooms were dingy. The towels were old and grayish and it looks like the soap may have been used before. Thin, cheap rug covered the floorboards. No TV, as advertised.
The breakfast was quite bad. Stale bread with no attractive spreads to speak of. Juice made from powder. One kind of sliced meat and one kind of cheese. Yogurt was fine.
The Ugly
The service was atrocious. When I first arrived, noone was at the desk. When she came, I went up to an office, where she mocked my language skills (I was operating in Russian), took phone calls and other guests ahead of me, actually yelled because I didn't have a print out of the reservation, and made insulting gestures.
