First drive to this cave yourself. You do not need a tour as the roads are fine and the cave well marked. Get there early and get Elvis as a guide. This trip was really great. Be advised that when we went (5/2013) the area with ridiculous amounts of guano was sealed off, i think due to alot of the more poor Trip Adviser reviews (every tour business heeds Trip Adviser in Costa Rica and you will be asked to give good ratings if you enjoyed the tour). The guide was good. Many people in my group backed out, however, as they were afraid of the closed spaces and the guide initially scared them a bit. I would suggest wearing long pants as you'll be crawling some.

This was a ton of fun, and we learned a lot. This is not like any kind of cave tour you might go on in the US. It's much more adventurous while being pretty tame and safe. You should definitely check this our if your in the area.
